Publicity Permission

I would love to post photos on our classroom blog. Also toward the end of the year, I make a DVD of photos set to music. I would like to distribute a copy to each of the students but I need to have your permission to do that. Please take a few minutes to complete the release agreement and have your child return it to school as soon as possible. I am unable to distribute DVDs until have a response from everyone.
